A Design Group, LLC, was retained by The Department of Veteran Affairs to provide Life Safety Consulting Services and review the Life Safety Compliance as related to The Joint Commission’s (TJC) Statement of Conditions (SOC) for the Louis A, Johnson VA Medical Center (LAJVAMC) located in Clarksburg, West Virginia.

A site visit was conducted by Rock Meng, PE, Principal of Capital Fire Protection, LLC and Frank Sturgeon, AIA (A Design Group, LLC). From that visit, this report, along with verified and updated Life Safety Plans and noted findings, was prepared. The verified and updated Life Safety Plans can be used for any regulatory survey, including The Joint Commission (TJC), VA Compliance, and other consultants. In addition, the Life Safety Plans should be consulted prior to any renovations, no matter how small, and provided to architects and design professionals prior to all major projects. A total of 150 findings were documented as part of our site visit.

This information will serve as the tri-annual Life Safety survey for the facility and is an important step in continued regulatory compliance. Regulatory excellence can only be maintained with continued attention to all aspects of regulatory compliance. This document is a tool to maintain the life safety compliance of the facility
